Fiona Miller (36), from Crossgates, is the second person to have lost her life on the A909 just outside Cowdenbeath since the start of the year.
She leaves behind two children, including a four-year-old daughter who was in the vehicle with her but was unharmed.
Mrs Miller died in hospital on Wednesday having been airlifted to Edinburgh Royal Infirmary after her people carrier collided with an articulated lorry on Saturday February 4.
Her family have asked for privacy to grieve but her husband Steve gave a brief tribute.
He said: ''Fiona was a loving wife, daughter, sister and most of all a loving mother to her two children. She will be sorely missed by her family and friends.''
An air ambulance, a rapid response crew, two ambulances and two fire engines attended the accident, which happened at around 11.30am.
Police have appealed for witnesses.
The same stretch of road claimed the life of Mossmorran worker Andrew Dobie (64), of Kirkcaldy, last month.
Mr Dobie was on the way to work when the head-on collision with another car happened. He died at the scene.
The other driver was uninjured. It is understood the accident may have been caused by black ice.
